5-Fluorouracil & Calcipotriol Combination Cream - A Snapshot

- Combination use first reported in January 2017 in The Journal of Clinical Investigation by Shadmehr Demehri et al.

- They explored calcipotriol as an inducer of thymic stromal lympopoietin (TSLP) which has strong antitumor activity.

- They performed the first study which was a randomised trial of calcipotriol combined with 5-fluorouracil for the treatment of actinic keratoses in 131 participants.

- It was found that after twice daily use for 4 days, calcipotriol plus 5-FU versus Vaseline plus 5-FU led to an 87.8% versus 26.3% mean reduction in the number of actinic keratoses in participants (P < 0.0001).

- Since then, several other studies and reports have been published supporting the use of the combination cream.

- Its use in immunosuppressed patients has was also published in Jan, 2023. In a patient-centred survey report of 32 immunosuppressed patients who had previously used 5FU monotherapy alone, 81% found 5FU/calcipotriol was more effective, and overall 87% preferred use of the combination cream over 5-FU alone.

- Reports on the prevention of skin cancers in patients with Xeroderma Pigmentosa has also been reported.

- Use of 5FU/calcipotriol cream is also reported for plane warts which can often be challenging to treat.

- For plane warts involving dorsal hands, it was used twice daily for 10 days. Reactions were mild erythema only. Complete clearance was achieved in 3 weeks.

- For plane warts on the face, the combination cream was used twice daily for 4 days. Complete response was seen in 3 weeks. Nil adverse reactions were reported.
